The episode kicks off with a lively OB-Gyn team dinner, filled with stories of old memories, funny nicknames, and laughter. Meanwhile, Yi-young, who had passed out from drinking, wakes up groggy in the restaurant restroom. Still dazed, she spots some dried pollock on a table and begins snacking, thinking it was leftover from their gathering. Only after a few bites does she realise, in horror, that she’s eating a stranger’s order—because their team dinner had long ended. Flustered, she apologises profusely, unaware that the man she’s apologising to is none other than Dr. Ahn Jeong-won (cameo by Hospital Playlist‘s Yoo Yeon-seok).

The next morning, Yi-young wakes up cringing as memories of the previous night come flooding back—especially the part where she blurted out all the evil nicknames they had for their senior professors. She tries to avoid Professor Seo at all costs, but to her surprise, Seo seems completely unfazed. Even when Dr. Eun-won tries to stir up drama by mentioning it, Seo Jung-min brushes it off casually. In a show of surprising maturity, Yi-young also doesn’t take the bait, leaving Eun-won visibly frustrated.

Elsewhere, Jae-il faces his own mini-crisis. While examining a pregnant patient, he mistakes a stool for an ovarian cyst, prompting Eun-won to reprimand him harshly. Just as things are about to get worse, Do-won steps in and takes the blame, shielding Jae-il from deeper trouble. Yet, there’s something suspicious—every time Do-won steps up, Eun-won seems a little too accepting. Is there a secret between them? Or something more?

Meanwhile, Sa-bi is working with a senior doctor on a case involving an elderly woman named Ok-sun with a uterus issue. After a long day, she’s exhausted but quickly cheers up at the news of food and rushes to join the other residents. In their room, Nam-kyung is glumly glued to her phone, hoping her ex-boyfriend will call after their breakup. When the others tease her, she puts on a brave face, but Yi-young senses something deeper when Nam-kyung talks awkwardly to her mother over the phone.

Soon, Yi-young is called to step in after an intern fails his duties. While managing patients, she snaps at the chatty intern, but Do-won gently advises her to be patient—after all, the boy is still very new. That night, as Yi-young and Do-won walk home together, she checks her phone and realises it’s past midnight—her birthday. Do-won is the first to wish her and promises to grant her any wish she asks.

Seizing the moment, Yi-young playfully asks him to agree to dinner and a movie. Do-won agrees but suggests they make it a family outing. As the elevator ascends, Yi-young hesitantly asks if she can like him. After a pause, Do-won says yes—but warns that if she wants things to get awkward between them, she’s welcome to proceed. It’s a bittersweet, quietly charged moment between the two.

The next morning, Yi-young celebrates her birthday with her sister, brother-in-law, and Do-won. During a phone call with her father, however, her disinterest is palpable, hinting at a strained relationship.

Back at the hospital, the first-year residents surprise Yi-young with gift cards, admitting they didn’t have time to buy real presents. Jae-il busies himself monitoring baby bumps but annoys the second-year residents by constantly double-checking his work. Meanwhile, Yi-young handles two cancer patients and reflects on their children, both the same age but vastly different in demeanour. One child is mature beyond her years, quietly carrying the burden of her mother’s illness, while the other remains blissfully innocent.

As Nam-kyung becomes increasingly obsessed with her phone and waits for a text from her ex, Sa-bi struggles to get Ok-sun’s consent for surgery. The elderly patient is hesitant, and Sa-bi’s first attempt to persuade her falls flat. Her senior encourages her to give it another try. Later, Jae-il asks Yi-young to deliver a document to Do-won. She eagerly agrees but finds Do-won pretending to sleep when she arrives. Disappointed but understanding, she quietly leaves, believing he must be exhausted.

Meanwhile, Do-won has a serious conversation with Jae-il over coffee, scolding him for burdening the second-years just to validate his own insecurities. It’s a humbling lesson for Jae-il, who realises he needs to step up his responsibilities. That evening, the first-years gather for snacks, savouring a rare moment of peace. Just as Sa-bi comments on the quiet night, the dreaded Code Blue bell rings—signalling an emergency. One of Yi-young’s cancer patients has gone into critical condition. As doctors work to stabilise the situation, Yi-young is tasked with comforting the patient’s daughter.

Instead of offering false hope, Yi-young opts for brutal honesty. She holds the girl’s hand and shares her own story of losing her mother at a young age. She encourages the girl to cry, to be angry, and to feel whatever she needs without guilt. It’s a raw, deeply empathetic moment that reveals Yi-young’s depth of character. She even gives the girl her number, promising to be there whenever she needs someone to talk to.

Then, Dr. Ahn Jeong-won arrives—revealed as the NGO guardian who has been supporting the family. Yi-young quietly leaves them together, and the girl finally breaks down, sobbing into Jeong-won’s comforting embrace.

The episode closes on a deeply emotional note. Early the next morning, Yi-young hears the voice of her late mother calling out to her. She stumbles into the living room, only to find it empty. Overwhelmed by grief and memories, Yi-young finally lets herself collapse, sobbing as she confronts her long-buried inner child—and the profound loneliness she still carries inside.
